Office Hours:
8:30am - 4:30pm Monday through Thursday
8:30am - 4:00pm Friday
Office appointments on Tuesdays and Thursdays
Surgery days are Mondays, Wednesdays and Fridays

Appointments:
Please call our office to make an appointment in advance. We will mail you a packet of information. Please fill this out completely and bring it with you the day of your appointment. This packet will contain directions to our office as well as confirmation of your appointment. If you need to cancel your appointment, please call us as soon as possible.

Preparation:
After arranging your first appointment, please have any medical records or X-rays mailed to our office as far in advance as possible. If any of the pertinent information is not available in advance, please bring it with you to your appointment.

If your insurance requires a referral authorization from primary care physician, please have it in place at the time of your appointment. If this is not available on the day of your appointment, you will be asked to sign an insurance waiver saying you will be responsible for all charges not covered by the appropriate referral. If you are filing insurance through Worker's Compensation, please seek proper authorization from your employer prior to treatment.

Telephone Calls:
Our staff answers most telephone questions and will be happy to assist you. If you request to speak to your doctor, your message will be taken and the doctor will return your call.

Prescriptions:
Please call during regular office hours when your records are available. Prescriptions will not be filled after hours or on weekends. Be sure to call before Friday 1p.m. for refills.

Access to X-Rays & Medical Records:
Use of x-rays in orthopaedics is absolutely necessary for successful diagnosis. We keep exposure to a minimum and observe every safety precaution. X-ray film remains the property of your doctor. Copies may be obtained for a nominal fee. We will be glad to provide you a copy of your medical records. All that is required is your signature for authorization and adequate advance notice.

Fees & Payments:
We make every effort to minimize the cost of your medical care and therefore require copayments be made at the time of your appointment. We accept cash, checks, and most credit cards. We'll bill your insurance company, however, payment is the patient's responsibility. We'll help in every way we can to assist in handling claims.

Insurance:
We accept assignment from Medicare and participate in most Preferred Provider organizations associated with Sisters of Providence Health System.
